By combining onsite and telehealth services with a variety of health practitioners including nurses, GPs, allied health professionals and specialists, Access Aged Care is able to offer a co-managed model of care for residents.
Our Access Aged Care service provides you and your residents with an integrated clinical support team that is able to provide a strong focus on individual patient care and customer service.
Attends the home and works closely with the resident and their care team.
Oversees resident care. Manages admission, care plans, family conferences, daily healthcare needs.
Attends the residence, works closely with the Care Manager and GP, Manages Specialist appointments.
Telehealth consults are booked using our secure, easy-to use Access Telehealth platform.
