Making Aged Care a Priority.

At Access Aged Care, we work closely with the facility, GP, resident and their family to improve the delivery of healthcare and achieve better health outcomes.

By combining onsite and telehealth services with a range of health practitioners including nurses, GPs, allied health professionals and specialists, Access Aged Care is able to create a collaborative care environment that gives residents and their families peace of mind.

Why you should partner with Access Aged Care.

  1. Be part of a team
    You’ll be part of a team consisting of you (delivered both onsite and some telehealth), a Geriatrician (via telehealth), a Nurse (onsite), a Locum GP after hours (telehealth) and admin support from our office.
  2. Deliver scheduled care requirements
    Provide scheduled requirements for residents. This includes things like Admissions, Care Conferences, Medication Reviews, CMAs and Care Plan Contributions.
  3. Fully Supported
    We manage everything through our operations team and provide you with all the support you need.
  4. Onsite nursing teams
    Assist both GP and Geriatricians to enable organise, supported and efficient care consultations.
